In 2011, I was asked to assess the communication and swallowing abilities of a man who had complex physical and learning difficulties. At that time, he was in an inappropriate placement which could not meet his needs. He was withdrawn, with little expressive communication. I became part of a bespoke package of care, which included other specialist health professionals and a dedicated support team. Today, he lives in his own specially adapted home with 24 hour staff support. He loves being around other people, is much more communicative and happy! His life has been transformed. This would not have been possible without the professionalism, knowledge, perseverance and highly tuned negotiation skills of the case manager. quote

I would pass on my gratitude for the way our case manager has supported my daughter and I. She immediately made us feel at ease and continues to maintain contact at all times. My daughter often messages her to tell her of personal achievements and our case manager is always most supportive, patient and kind. Many Thanks. quote
